The UHP Dental Plan is the ideal solution for Americans who have no type of dental coverage or want to supplement existing coverage and maintain a beautiful smile. As a dental subscriber, you are entitled to the best dental care available and may therefore select any dentist listed in the directory. And, you may change dentists at any time for any reason!
Members Have Access to Dentists Under Three Distinct Scenarios:
1. At select participating Schedule A general practitioners around the country, members receive a No Charge exam and x-rays in conjunction with a paid annual cleaning* and a fee schedule of fixed payments for most procedures. Fixed schedule procedure rate savings are on average 30% to 60% savings on dental care as compared to American Dental Association surveys of usual and customary fees.

- 20% to 60% Savings on all other procedures
- Savings on check-up (oral examination, x-rays and cleaning) alone is typically more than the retail annual plan price in many areas of the country.

How to Access this Benefit:
- Choose the most convenient participating dentist from the provider locator. If you need help or have specific questions, simply call the toll free number on your membership card.
- Call to make an appointment. If you are visiting a Tier 1 or Tier 2 dentist, be sure to mention that your membership has access to the Dental Directory Services, DDS dental network. If you are accessing a Tier 3 dentist, please call the dental customer service number on your membership card for further instructions.
- Present your membership card each time you visit your dentist office. Your dentist may call the dentist services number to confirm eligibility.
- Pay your dentist directly, at your special member rates, and enjoy the savings!
