How to Access this Benefit: Simple as 1-2-3

1. Call the toll free number in your membership materials to locate a chiropractic provider location.
2. Call the Chiropractor's office to make an appointment and identify yourself as a UCP (United Chiropractic Program) member.
3. Present your membership card each time you visit your chiropratic office. Your chiropractor may call the chiropractic services number to confirm eligibility.
4. Pay your chiropractor directly (UHP does not make make payments directly to the provider), at your special member rates, and enjoy the savings!
